Social Work Coordinator

Job Title: Social Work Coordinator

Status:  Full-Time/Exempt/40 hours per week requiring flexibility

Compensation:   $55,000 plus generous paid time off and benefits package

Position Summary:

The Social Work Coordinator will primarily provide support, assistance, and follow-up for program participants (aka patients) who receive mental health services from PPP’s collaborative, van-based medical services project with AHN.

Additional responsibilities will include assisting with the logistical operations of the medical van, and supporting the medical providers as appropriate.  The mobile medical services project started in 2020 offering wound care, buprenorphine prescribing, and dispensing of antibiotics, over the counter medications, and supplements at PPP’s existing harm reduction service locations. Additional funding has been received to add low barrier mental health services and this position will focus on helping program participants find mental health services and guiding them through the continuum of care.

The position is funded by a 5 year SAMHSA grant.

Responsibilities:

  • Be present to staff the PPP medical van sites. Initially this will be every Wednesday and Friday but may expand to other days in the near future.
  • Collaborate with PPP and AHN staff to provide quality care to program participants and ensure compliance with grant requirements.
  • Meet with program participants who receive services from medical staff at the van sites, or other appropriate locations.  Work will take place primarily in the field, with minimal time spent in the office.
  • Provide support to participants, help assess their needs and provide assistance as necessary.  This may include scheduling appointments on the van, connecting and scheduling with other providers, assistance in completing medical assistance applications or other similar forms, overcoming transportation obstacles, and assisting in other areas of need that may arise.
  • Provide support for the operation of services, including driving the van and being responsible for van maintenance.
  • Maintaining adequate supplies and staffing for service sites. This includes cross-training to provide coverage for other PPP site roles.
  • Support and participate in regular discussions with contracted medical providers regarding the provision and coordination of care for current, future, and past patients.
  • Grow our  partnerships with individuals in other organizations in order to build relationships to refer participants to other levels of care.
  • Represent the agency in task forces, advocacy groups, and advisory bodies as appropriate.
  • Manage program data, including Protected Health Information with all appropriate protections. This also includes grant-related documentation such as mandated GPRA reporting.
  • Collaborate with the Service Navigation Specialist (and other staff) to receive and facilitate referrals to medical van services,  to identify and coordinate with outside organizations to bring additional resources to service locations and program participants, and to actively connect participants with other services as needed
  • To be genuine, engaging, and trustworthy when interacting with program participants.
  • To exemplify professionalism when working with partner agencies and authorities.
  • To be accountable and responsive to program participants, partners, coworkers, and agencies.
  • To function as a team-member, share in other responsibilities as required, including but not limited to: attending events, obtaining training, undertaking research, and other duties as assigned.
  • To maintain necessary licensing and training, and to adhere to ethics of social work, if applicable.
    Attend and contribute to regular staff meetings.
